A5: One-sided 7 inch cardboard picture (portrait of Lou by Billy Name on the play side) record, part of Andy Warhol's Index book, a multimedia package (1966). B1 to B4: rough mix demos from 1969, originally available on the EP [r=670929] (1976). B5: from [m=54737] (1966). There are at least three different editions: 1. First edition (1979) has a silk-screened cover. Left hand corner of the back cover reads:"design & silk-screen printing". ***this*** 2.
