Doublesided Xerox insert with lyrics on one side and staff/tracklist on the other side. Some copies came also with the later simplified text-only insert and a A2 sized poster of Madre Del Vizio. Lacquer cutting engineer and lacquer cutting studio uncredited, identified by the etched matrix.
